New surgery video: Dr. Tehrany performs arthroscopic rotator cuff repair

Rotator cuff injuries are prevalent shoulder injuries, and the risk of their development increases with age. However, rotator cuff injuries can also occur due to repetitive overhead motions or trauma to the shoulder, such as direct fall.
Dr. Armin Tehrany, an Honorary NYPD Orthopedic Surgeon, had a patient recently with a severe rotator cuff tear. The 46-year-old gentleman, an NYPD Officer, injured his shoulder while on duty and suffered a complete rotator cuff tear from the bone.
According to Dr. Tehrany, the patient was too young to experience an injury that severe. After a thorough examination, Dr. Tehrany recommended proceeding with a rotator cuff repair to mitigate the pain.
Once again, Dr. Tehrany took the opportunity to transform the surgical experience into an educational story. With the patient’s consent, Dr. Tehrany recorded the surgery, hoping it will help the viewers learn more about the arthroscopic rotator cuff surgery flow.
At the beginning of the surgery, Dr. Tehrany highlighted the crucial part of the surgery, which was a bony growth that inhibits shoulder mobility.
“This is a bone spur that impinges against this torn rotator cuff, so what I am doing here is polishing and smoothing that spur,” Dr. Tehrany explained.
Before the rotator cuff tear repair, Dr. Terany performed subacromial decompression to create free space between the bone and the rotator cuff. Additionally, he removed bone tissue and increased the open area between the ball-and-socket and the acromion.
For addressing the severe rotator cuff tear, Dr. Tehrany used the side-to-side shoelace technique. This simple technique has shown favorable outcomes for massive tears. It decreases margin strain and can easily incorporate into current transosseous-equivalent rotator cuff repair configurations.
“We are going to tie a knot from the anterior leaf here and connect it to the posterior leaf,” Dr. Tehrany explained in the video.
The rotator cuff tear was massive. However, as a remarkably skilled and experienced shoulder surgeon, Dr. Tehrany successfully repaired the patient’s shoulder arthroscopically.
